AMSEA'S EQUIPMENT LOAN TERMS - Please read the following terms and check the box below prior to submitting this form.

AMSEA will send equipment to you in good working condition. AMSEA's equipment gets a great deal of use throughout the year. In order to ensure its long life and availability to others in need, AMSEA requires you to agree to the following terms prior to borrowing equipment. By checking the box below, I commit to:

• Request equipment with as much advance notice as possible, realizing that equipment is loaned on a first-come, first-served basis.
• Report any damaged or defective equipment to AMSEA as soon as possible, regardless of cause.
• Contact AMSEA to request an extension if the equipment will be held longer than expected, understanding that a late fee may be charged for unapproved late returns.
• Return the equipment to AMSEA--DRY and packaged as it was received within 24 hours after use or the first available opportunity for shipping.
• If I fail to return any items borrowed from AMSEA BY 30 days past date due, I am liable for replacement costs of that item/items.(See Last Item)
• INSURE the equipment with the return shipper (AMSEA will provide the value if needed).
• Pay for return postage/shipping and insurance.
