AMSEA Video Check-Out Form

AMSEA's lending library includes videos, listed below, that are available for use in cold water and outdoor safety training by AMSEA-certified instructors.

Please read all information, instructions and library loan terms on this page before placing an order for videos.

Only AMSEA-trained instructors may borrow videos.

To borrow AMSEA's cold water safety and survival videos for use in safety training, fill in your name, contact information, and information about your upcoming training. Also, you must read the AMSEA Library Loan Terms and indicate your acceptance of them.

Then use the quick links below or scroll down the form, and check the boxes next to the items you'd like to borrow. Finally, click the "Submit Form" button at the bottom of the page.

Once we receive your request, we will confirm availability by e-mail or phone and ship the videos to you. You are responsible for shipping all items back to AMSEA at your expense.

PLEASE NOTE: As a borrower, YOU (and no one else) are responsible for returning items in the same condition as you received them.

As a borrower, YOU are responsible for returning items in the same condition as received.

AMSEA will send videos to you in good working condition. AMSEA's videos see a great deal of use throughout the year. In order to ensure their long life and availability to others, AMSEA requires you to agree to the following terms prior to borrowing videos.

AMSEA Library Loan Terms

By checking the box below, I agree to:

• Request books and/or videos with as much advance notice as possible, realizing that items are loaned on a first-come, first-served basis.
• No more than 10 books or videos may be borrowed at one time.
• Maintain physical control of the borrowed items at all times.
• Report any damaged or defective items to AMSEA as soon as possible, regardless of cause.
• Contact AMSEA to request an extension if the items will be held longer than expected, understanding that a late fee may be charged for unapproved late returns.
• Return items to AMSEA packaged as they were received.
• Ship items back to AMSEA within 24 hours after use or the first available opportunity for shipping.
• Pay overdue fees of $1.00 per day per item for books or videos returned after the due date.
• If I fail to return any items borrowed from AMSEA more than 30 days past date due, I am liable for replacement costs.
• INSURE the items with the return shipper (AMSEA will provide the value, if needed).
• Pay for return postage/shipping and insurance.
