Resources for AMSEA-Trained Instructors:
We rely on training statistics from you to obtain the grant and contract funds that keep AMSEA operating, supporting you and other cold-water safety instructors. This funding allows us to promote courses, send training equipment and other supplies, provide curriculum, support instructor training, sponsor traveling instructors, and pay overhead expenses. So please help AMSEA continue our life-saving mission by submitting the information needed by our funders.
USCG REQUIREMENT: Reporting Needed BEFORE You Provide Training!
A U.S. Coast Guard directive requires AMSEA to notify the USCG of classes before they happen. We now receive funding only as a result of training statistics from classes that the USCG is informed of before they take place.
Our directive is to supply information to the USCG at least 30 days before any class. However, if you cannot make the 30-day deadline, please submit your information any time – but as far in advance as possible – before the class.
Please use AMSEA's new pre-course reporting form to provide us information about the training you plan to offer:

Additional information will also be requested from you after you teach a class. When we receive your pre-course information, we will provide you with the forms and instructions for post-course reporting .

Download this PDF Student's Course Evaluation Form, print a copy for every student age 18 and over (age 16 and over for F/V Drill Conductor classes), and have them complete the form. Please mail or fax completed forms to AMSEA.

Going To Teach a Fishing Vessel Drill Conductor Course?
Instructor's List of Supplies & Equipment for Teaching Survival Equipment, Procedures and Onboard Drills Courses: We have created a list of needed teaching supplies and equipment for instructors teaching AMSEA’s USCG-accepted 10-hour or 18-hour drill conductor classes. This list helps you gather the required paperwork, decide what equipment to borrow from AMSEA and serves as an inventory of the supplies you need to gather.
Forms Needed for Teaching Some Courses

Other needed forms, as indicated below, will be sent to you upon submission of a Pre-Course Reporting Form (see top of page).
Sample Assumption of Risk and Release Form Must be completed by all students in courses taught by instructors working for AMSEA in any capacity - as employees, contract instructors, or volunteers. Completed forms should be submitted to AMSEA.
Assumption of risk and release forms are strongly recommended to all instructors.
Drill Conductor Course Skills Checklist (Print landscape). Must be completed and returned to AMSEA whenever Drill Conductor courses are provided.
AMSEA Course Roster/Reporting Form: A roster must be completed and returned to AMSEA for most courses with adult particpants.
Appropriate course roster forms will be provided to instructors upon submission of a Pre-Course Reporting Form (see top of page). Completed rosters must be submitted to AMSEA.
Alaska Water Wise Forms: Please contact the Alaska Office of Boating Safety (visit their website or telephone 907-269-8705) for current roster, skills checklist, and course evaluation forms. Please report Alaska Water Wise courses that you teach to AMSEA using our easy, online post-course reporting form.

Code of Conduct for AMSEA directors, staff, volunteers and instructors under contract.
Incident/Accident Report - Should anyone become injured, any damage occur or any other reportable incident take place during AMSEA-sponsored training, this report must be completed and submitted to AMSEA's Executive Director.
Drill Conductor cards: Instructors who teach AMSEA Drill Conductor courses issue the students who successfully complete those courses a Drills Card that serves as proof of compliance with USCG training requirements for fishing vessels that fish outside the Boundary Line. The cards currently are valid in perpetuity.
Your former students who lose their Drills Cards may be directed to AMSEA for replacements. See our Drills Card page for additional information.
